Cancellation Auto-Renewal Terms:

• Cancel by contacting office.
• Monthly membership fees are non-refundable.
• A minimum 12-month commitment is required unless proof of moving more than 50 miles outside of the area.

Approved early cancellation requires payment at full price for any used units rendered during my membership term. To receive discounted pricing online during your membership, you must be logged into your account on the store and follow the instructions during checkout. Service options and some cosmetic treatments discounts are only available in the office.
